we, the undersigned, made this act of the characteristics and condition of the apartment which №4, according to warrants from 20 December 2012 g. (a photo F0), Shevchenko filled DAM g. Kiev,
according to the decision from 19.04.2012g. Kyiv City State Administration of allocation habitation in Svyatoshinsky district g. Kiev Street. Klavdievskaya 40-B, on the occasion of 26 anniversary of the Chernobyl disaster, citizen Varivoda Anatoly Iosifovich,
registered in the area Desnyanskiy g. Kiev, referred to the 1 categories of persons, who have suffered as a consequence of the Chernobyl disaster, victim identification (category 1) Series A №425987.
